Our learning disability team support our adult patients with a learning disability and/or autism, their families and carers through their hospital journey. This could include:
- Supporting and promoting the use of the hospital passport or purple folder that holds information about the person
- Facilitating reasonable adjustments to support access to care and treatment for our patients. Reasonable adjustments could include support with clinic appointments, blood tests, investigations and diagnostic appointments
- Supporting people with learning disability and/or autism during their stay in hospital
- Liaising closely with our patient’s next of kin, relatives, paid carers and the community learning disability teams
