Our learning disability team support both children and adults with a learning disability and/or autism their families or carers through a hospital journey.
Our team consists of Lead Nurse, Learning Disability Assistant Practitioner and Learning Disability Assistant. Our working hours are Monday to Friday from 8am to 4pm.
The team will:
- Support and promote the use of hospital passport on our wards. These hold crucial information about an individual with a learning disability that all staff should be aware of
- Regularly review patients who are on our wards, please ensure we are made aware of any admissions by e-mailing our team e-mail [email protected]
- Liaise with professionals ensuring Mental Capacity assessments are carried out where necessary and best interests decisions are made
- Liaising with community teams to support with any elective procedures
- Ensuring reasonable adjustments are facilitated whilst in hospital to improve overall health outcomes and reduce instances of diagnostic overshadowing
- We can support individuals/families/carers to clinic appointments, outpatient appointments, investigations, diagnostic appointments etc
