The West Essex place has a number of community based urgent care services in addition to local pharmacies and GP surgeries. The ambulance service, NHS 111 and GPs have details of these services and access them through one phone call with the Care Co-ordination Centre. They are:
Urgent Community Response
Urgent community response gives a two-hour response to urgent needs in a patient’s home or in a community hospital. It also takes appropriate calls from the ambulance queue and from paramedics at a patient’s home. They can respond to physical, mental health and or social care needs.
Falls response vehicle
Falls response vehicle staff will attend adults who have fallen and assess for injury, lift them appropriately and put in place further care services as required.
Hospital at Home
Hospital at Home is personalised care at home for adults from a team of medical consultants, clinical practitioners, physical and mental health nurses, healthcare support workers and administrators. Care is either face to face or remote monitoring and equipment is provided. Patients are reviewed daily through a multi-disciplinary ‘board round’ and informed of their progress through their care giver.
The Bridging Service
The Bridging service provides short-term intensive social care support to adults at home to avoid being admitted into hospital and to support discharge from hospital. The service maximises independence, which supports mental wellbeing and improved physical condition.
The four community wards
The four community wards take direct admissions for urgent care and step-down admissions from acute hospitals to give care that cannot be given at home and patients that require medical and therapy rehabilitation.
