Who qualifies for Ambulance Transport?
- Stretcher Patients
- Wheelchair Users
- Patients with Learning, Communication or Psychiatric difficulties, who are unable to use public transport
- Patients with a medical condition that would compromise their dignity or cause public concern
- Patients who cannot walk without the continual support of a walking aid (i.e. Zimmer frame)
- Patients who may experience side effects due to the treatment the receive
If you are not able to travel on public transport due to a medical condition (stated above) your GP may make a request for Ambulance Transport.
This request must still comply with PAH Trust’s ‘Non-Emergency Patient Transport’ criteria.
Once you have used hospital transport if you require further transport for the next appointment please call 01279 827191.
Please remember to be ready for transport 2 hours before your appointment time.
NHS Travel Costs
You can get help with necessary cost of travel to receive NHS treatment under the care of a Consultant if you:
- Are a war pensioner and the treatment is for your accepted disablement
- Are getting, or your partner gets (and includes travel by your dependant children);
- Income Support
- Income-based Jobseeker's Allowance (Incapacity Benefit or Disability Living Allowance do not count as they are not income-related)
- Pension Credit Guarantee Credit
- Are entitled to, or named on, a valid NHS credit exemption certificate.
- Are named on a valid HC2 certificate
Reimbursements will only be issued with proof of the above.
Cancellations
If your appointment has been changed or you can not attend it please cancel your transport immediately.
Please contact your GP or hospital doctor to request hospital transport for your new appointment.
