Going Home and Follow Up Appointments
A discharge date is set before you come into hospital according to when your care and treatment is due to finish.
The ward team will work alongside you to plan your discharge from hospital. The details are written in a document called a core discharge plan.
Any continuing health and social care needs will be assessed and you will only be allowed to go home once these arrangements have been made.
All your discharge arrangements, such as name and telephone number of any services organised for you, will be written on the core discharge plan.
Copies of this document will be sent to your GP and Community Services (if you are receiving them) alongside a letter explaining your treatment.
Follow Up Appointments
Inpatients
Your doctor will let you know if you require a follow up appointment. The ward clerk will normally book this before you leave.
Outpatients
Once you have seen the Consultant or a member of his/her team they will hand you a document.
On leaving please hand to the receptionist who will advise the process for booking another appointment if you are required to do so.
